Passports applications are accepted in the following locations:
Kingman - 8:00am - 5:00pm Monday through Friday
Lake Havasu City - 8:30am - 12:00pm and 1:30pm - 4:30pm, Monday through Friday
Bullhead City - 8:30am - 12:00pm and 1:30pm - 4:30pm, Monday through Friday
To submit a DS-11 Passport Application for an adult you must present the following:
- Proof of U.S. Citizenship (bring in an original AND one photocopy of your birth certificate or naturalization certificate.)
- Proof of identity. (bring in your valid driver’s license AND also a photo copy of the front of back of your driver’s license.)
- One recent color passport photograph.
An out-of-state primary ID, such as a driver's license, also requires a second ID
(Submit a front and back photocopy of that second ID.)
To submit a DS-11 Passport Application for a child under age 16, both parents or the child’s guardian(s) must appear and present the following:
- Proof of the child’s U.S. Citizenship (bring in an original AND one photocopy of the child’s birth certificate or naturalization certificate.)
- Evidence of the child’s relationship to the parents/guardian(s).
- Parental/Guardian identification (bring in your valid driver’s license AND also a photo copy of the front and back of your driver’s license); One recent color passport photograph.

(Submit a photocopy of the front and back of that second ID)
If only one parent appears, you must also submit a second parent’s notarized written statement consenting to passport issuance for the child, primary evidence of sole authority to apply, or a written statement (made under penalty of perjury) explaining the second parent’s unavailability.
Each minor child applying for a passport book and/or passport card must appear in person.
To submit an application if you were born outside the United States you much present a Certificate of Naturalization of your parent(s), your foreign birth certificate and proof of your admission to the United Sates for permanent residence, and Proof of identity (previous U.S. Passport, driver’s license and a photo copy of the front and the back of your driver’s license).
If you are submitting anything other than a U.S. passport, please provide a photocopy of each document and one recent color passport photograph.
Note: Applying for a Book and Card requires 2 color passport photographs.
