Passports

Passports applications are accepted in the following locations

Kingman - 8:00 am - 5:00 pm Monday through Friday

Lake Havasu City - 8:30 am - 12:00 pm and 1:00pm - 4:30pm, Monday through Friday

Bullhead City - 8:30 am - 12:00 pm and 1:00pm - 4:30pm, Monday through Friday

What do I need to submit?

To submit an application for an adult you must present the following:

Proof of U.S. Citizenship
Proof of identity (Bring in your driver's license and also a photo copy of the front and back of your driver's license)
Two recent color photographs, AND
Fees (see chart below)

To submit an application for a child under age 16 both parents or the child's guardian(s) must appear and present the following:

Evidence of the child's U.S. Citizenship
Evidence of the child's relationship to parents/guardian(s), AND
Parental/Guardian identification (Bring in your driver's license and also a photo copy of the front and back of your driver's license)

If only one parent appers you must also submit one of the following:

Second parent's notarized written statement consenting to passport issuance for the child,
Primary evidence of sole authority to apply, OR
A written statement (made under penalty of perjury) explaining the seconed parent's unavailability

Each minor child applying for a passport book and/or passport card must appear in person.

To submit an application if you were born outside the United States you must present the following:

Certificate of Naturalization of your parent(s), your foreign birth certificate and proof of your admission to the United States for permanent residence
Proof of identity (previous U.S. Passport, driver's license and a photo copy of the front and back of your driver's license)
Two recent color photographs

The $25.00 Execution Fee is not charged when adults 16 and over with fully valid passport books apply for the passport card using a DS-82 form.
